Why add glucagon?

Glucagon is best known for raising blood glucose, which makes it an unexpected partner in metabolic research. But glucagon-receptor signaling also increases energy expenditure and promotes fatty-acid oxidation in the liver. Pairing it with GLP-1 and GIP activity, which counter its glycemic effect, is the rationale behind tri-agonism 1.

How the molecule is built

Retatrutide is based on a GIP peptide backbone, modified to also activate GLP-1 and glucagon receptors, and conjugated to a fatty diacid moiety that binds albumin in circulation. What the clinical data showed

In a 48-week phase 2 trial in adults with obesity, published in the New England Journal of Medicine, retatrutide produced dose-dependent reductions in body weight, reaching a mean of 24.2% in the highest-dose group versus 2.1% with placebo 2. A separate phase 2 trial in type 2 diabetes reported meaningful reductions in HbA1c and body weight 3. In a substudy focused on liver fat, a majority of participants in the higher-dose groups reached liver-fat levels within the normal range by 48 weeks 4.

Tri-agonism does not just add a third pathway. It changes the balance between eating less and spending more.

Safety signals observed

As with other incretin-based compounds, the most frequent adverse events in trials were gastrointestinal (nausea, diarrhea, vomiting), largely mild to moderate and more common during dose escalation. Dose-related increases in heart rate were also reported 2. These observations come from controlled clinical settings with medical supervision.

What remains open

• How much of the weight lost is fat mass versus lean mass, and how that compares across agonist classes.
• Durability of effects and what happens after treatment stops.

KPV: calming inflammation

KPV (Lys-Pro-Val) is the C-terminal tripeptide of alpha-melanocyte-stimulating hormone (α-MSH), a molecule with known anti-inflammatory activity. Why study them together?

In cultured human somatic cells, it was reported to induce telomerase activity and telomere elongation 2. These findings come largely from a single research group and have not been widely replicated, which is why independent confirmation remains a priority.

MOTS-c: a signal from the mitochondria

MOTS-c is a 16-amino-acid peptide encoded within mitochondrial DNA, part of a newer class of mitochondrial-derived peptides. In mice, it acted on skeletal muscle to improve insulin sensitivity and protected against diet-induced metabolic dysfunction, partly through AMPK activation 3. Its natural levels decline with age, which places it at the intersection of mitochondrial function and nutrient sensing.

Growth hormone (GH) is not secreted at a steady rate. The pituitary releases it in bursts, most prominently during deep sleep, and this pulsatile pattern is part of how tissues read the signal. Growth hormone secretagogues are compounds that stimulate those pulses rather than supplying the hormone itself.

The three regulators of the GH axis

• GHRH (growth hormone–releasing hormone), from the hypothalamus, stimulates GH synthesis and release.
• Somatostatin inhibits release, creating the troughs between pulses.
• Ghrelin, acting on the GHS-R1a receptor, amplifies pulses and counteracts somatostatin.

GH then acts on the liver and other tissues to produce IGF-1, which mediates many of its growth and metabolic effects and feeds back to dampen further release.

Ipamorelin: selectivity as a feature

Early ghrelin mimetics also raised cortisol and prolactin. Ipamorelin was described as the first selective GH secretagogue because, in animal studies, it released GH without significant increases in ACTH or cortisol, even at doses well above those needed for GH release 1. That selectivity is why it is often chosen in research designs that want to isolate the GH axis.

CJC-1295 and the DAC modification

Native GHRH and Sermorelin are cleared within minutes. CJC-1295 with DAC (Drug Affinity Complex) carries a reactive group that binds covalently to circulating albumin, dramatically extending its presence in the body. In healthy adults, a single administration produced sustained increases in GH and IGF-1, with an estimated half-life of roughly 6 to 8 days 2.

Tesamorelin and visceral fat

Tesamorelin is a stabilized GHRH analog that has been studied in randomized controlled trials. In patients with HIV-associated abdominal fat accumulation, it reduced visceral adipose tissue while preserving subcutaneous fat, alongside increases in IGF-1 3. It is one of the most clinically characterized molecules in this class.

Secretagogues work with the body's rhythm instead of overriding it. That is both their appeal and the reason pulsatility belongs in every research design.

Why GHRH and ghrelin mimetics are studied together

Because GHRH analogs and GHS-R1a agonists act on different receptors, their effects on GH release can be synergistic: the GHRH signal drives the pulse while the ghrelin-like signal amplifies it and blunts somatostatin's brake. How strong is the evidence?

Both compounds have a substantial literature, but much of it originates from a limited number of research groups, and large independent randomized trials are scarce. For researchers, that is a reason for careful design rather than dismissal: clear behavioral or molecular endpoints, appropriate controls and transparent reporting are what will move the field forward.

1. Start with a falsifiable question

"Does peptide X help recovery?" is not testable. "Does peptide X increase fibroblast migration in a scratch assay compared with vehicle at 24 hours?" is. A precise question defines the model, the comparison, the endpoint and the time point all at once.

2. Choose the right model

Cell lines are fast and controllable but simplified. Primary cells and tissue explants add realism at the cost of variability. Animal models capture whole-organism effects and come with ethical obligations and regulatory oversight. Pick the simplest model that can actually answer your question.

3. Define controls

• Vehicle control: the same solvent, volume and handling, without the peptide.
• Positive control: a compound with a known effect, proving the assay can detect one.
• Single-component controls when studying blends, to see which ingredient drives the result.

4. Randomize and blind

Assign samples or subjects to groups randomly, and keep the person measuring outcomes unaware of group allocation. These two steps remove the most common sources of unconscious bias, and they are among the items most often missing from published studies 1.

5. Decide the sample size in advance

Underpowered studies produce noisy results that are hard to interpret and easy to over-read. Use a power calculation based on the effect size you expect and the variability of your assay, and state the number before you start.

Every research peptide is sold with a number attached to it, usually something like ≥98% or 99%. That number is only meaningful if you know how it was produced. A purity figure is the output of an analytical method run on one specific batch, on one specific date, by one specific laboratory. Change any of those and the number can change too.

Purity is a measurement, not a label

Synthetic peptides are built one amino acid at a time, usually by solid-phase peptide synthesis. Each coupling step is highly efficient but never perfect, so every crude batch contains small amounts of closely related molecules: sequences missing one residue (deletion peptides), truncated chains, incompletely deprotected side chains, oxidized or deamidated variants 2. Purification removes most of them. The purity figure tells you how much remains.

That is why a reputable supplier certifies each batch individually. Two vials of the same compound from different syntheses are, analytically speaking, two different samples.

What HPLC measures

High-performance liquid chromatography (HPLC), almost always in reverse-phase mode, separates the components of a sample by how strongly they interact with a hydrophobic column. As the sample travels through, each component exits at a characteristic retention time and is detected by UV absorbance, typically around 214–220 nm, where the peptide bond absorbs strongly 1.

The result is a chromatogram: a trace with one dominant peak (the target peptide) and, ideally, only small peaks around it. Purity is reported as area percent: the area of the main peak divided by the total area of all detected peaks.

What mass spectrometry adds

A sample can be 99% pure and still be the wrong compound. Mass spectrometry (MS) measures the molecular weight of what is in the main peak and compares it with the theoretical mass of the intended sequence. When the observed and theoretical masses match within the instrument's tolerance, identity is confirmed. HPLC answers how much; MS answers what.

How to read a COA in five checks

• Batch or lot number that matches the number printed on the vial.
• Method stated explicitly (e.g. RP-HPLC with UV detection; ESI-MS for identity).
• Chromatogram attached, not just a number typed into a table.
• Observed vs. theoretical mass reported side by side.
• Laboratory name and test date, ideally an independent third party.

Why the number changes from batch to batch

Small variations between syntheses, purification runs and lyophilization cycles are normal. A batch certified at 99.2% and the next at 98.9% is not a red flag. What would be a red flag is a COA that never changes, carries no batch number, or cannot be traced to a laboratory. Identical numbers across many batches usually mean the document is generic.

A purity number without a batch number is a marketing claim. With a batch number, a method and a chromatogram, it becomes evidence.

Three questions to ask before you trust a formulation

• Does this COA correspond to the exact batch I am receiving?
• Were both purity (HPLC) and identity (MS) tested?
• Who performed the analysis, and can I see the raw data?

Why peptides degrade in solution

Once dissolved, peptides are exposed to several well-described degradation pathways 1:

• Oxidation of methionine, cysteine and tryptophan residues, accelerated by light and dissolved oxygen.
• Deamidation of asparagine and glutamine, especially in Asn-Gly sequences, accelerated by heat and higher pH.
• Aggregation, often triggered by agitation and air–liquid interfaces.
• Adsorption to glass and plastic surfaces at very low concentrations.

Choosing the solvent

For most research peptides, the standard choices are sterile water or bacteriostatic water (sterile water with 0.9% benzyl alcohol as a preservative) 2. Bacteriostatic water is preferred when a vial will be accessed more than once, because the preservative limits microbial growth between uses. A few hydrophobic sequences require small amounts of other solvents; the supplier's technical data should guide that choice.

Step-by-step reconstitution

• Let the sealed vial reach room temperature before opening, to avoid condensation inside.
• Wipe the rubber septum of both vials with 70% isopropyl alcohol and let it dry.
• Draw the solvent with a sterile syringe and inject it slowly down the inner wall of the vial, not directly onto the powder.
• Swirl or roll gently until the cake dissolves. Never shake: shear and foaming promote aggregation.
• Inspect the solution. It should be clear and free of particles.
• Label the vial with compound, concentration, solvent and date.

Calculating concentration

Concentration is simply mass divided by volume. A vial containing 10 mg of peptide reconstituted with 2 mL of solvent yields a 5 mg/mL solution. Adding 1 mL instead yields 10 mg/mL. Record the value on the label and in your lab notebook so every downstream calculation in the experiment starts from the same number.

Storage and aliquoting

Before reconstitution, keep vials sealed, protected from light and moisture, refrigerated for routine use or frozen at −20 °C for long-term storage. After reconstitution, keep the solution at 2–8 °C, away from light. If a solution will be used over a long period, divide it into single-use aliquots so the main stock is not repeatedly warmed, punctured or frozen and thawed.

Why levels fall with age

In multiple animal models and some human tissues, NAD+ levels decline with age. One well-supported driver is increased expression of CD38, an NAD+-consuming enzyme that rises with age and inflammation; in mice, CD38 deficiency preserves NAD+ levels and mitochondrial function 3. Chronic DNA damage, which keeps PARPs active, adds further demand.

The question is no longer whether NAD+ can be raised. It is whether raising it changes anything that matters.

Precursors vs. NAD+ itself

Research has explored several routes to raise NAD+: precursors such as nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN), direct NAD+, and inhibition of NAD+-consuming enzymes 2. Each has different uptake routes and tissue distribution, and comparisons between them are still an active area of study.

What human studies show so far

In a randomized, placebo-controlled crossover trial in healthy middle-aged and older adults, six weeks of NR was well tolerated and raised NAD+ in peripheral blood mononuclear cells by roughly 60% 4. Effects on blood pressure and arterial stiffness were suggestive but not definitive. Across the broader literature, biomarker changes are consistent; functional outcomes such as muscle performance or insulin sensitivity have been mixed.
